2026 Membership and Rules Forms

Please download and fill out the 2026 membership application and Rules. Sign and date the organization rules and membership application. Please note there will be 2 pages to SIGN then mailed or emailed to POMC.
Mail the membership application and organization rules to the address contained at the bottom of the membership form. Please make sure that everything is filled out and signed or your application processing may be delayed.

CLUB MEMBERSHIP IS SUBJECT TO POMC BOARD APPROVAL.
NON MEMBERS
SHOULD NOT SEND ANY MONIES WITH A SUBMITTED APPLICATION REQUEST.
